All applicants (except for Mentor of the Year category) must be aged 35 or under on 8 November 2013.

All completed and returned entries will be acknowledged online upon completion. If you do not see an acknowledgement then please contact Sarah Collis at Don't Panic on 01706 828855 to confirm receipt of your entry. The organisers do not accept any responsibility for loss or damage of any entry.

All entry payments must be made by Friday 29 November in order for entries to be processed.

Please ensure you read the supporting notes for the awards you are entering. Remember to use evidence such as quotes, statistics and relevant URLs and not to exceed the suggested word count of 1000 words.

Please complete all sections of the form, entries that are incomplete or exceed the stated word limits will not be accepted.

All nominees must be available to attend an interview on Wednesday 8 January 2014 should their nomination be shortlisted. Shortlisted nominees will be informed of this and will recieve their invitation to interview on Thursday 5 December 2013.

The judges’ decision is final and no correspondence or discussion will be entered into or feedback given regarding the selection of short listed nominations or winners.
